Under the general direction of the Housing Director, the Case Manager will provide intensive case management services to formerly chronically homeless persons living at Palo Duro I to achieve increase participant stability and self-sufficiency.  This position will be responsible for establishing close cooperative relationships with local shelter, hospital and law enforcement to identify and recruit perspective participants.  The incumbent will also follow established procedures to determine eligibility, monitor client progress and needs while living in Palo Duro I and coordinate resource services as needed.

Essential Functions

  • Provide intensive case management services to approximately 22 clients. 
  • Explain rules, policies, and regulations to tenants. 
  • Establish and maintain collaborative working relationships with homeless shelters and community resources.
  • Prepare required monthly reports.
  • Attend appropriate coalition and other community meetings as assigned by supervisor.
  • Promote goals of the organization through tours and presentations.
  • Maintain consistent feedback to the Housing Director regarding all major areas of programs functioning.
  • Ensure compliance with OCHA policies, fair housing policies, the Landlord Tenant Act, and NSO requirements at all times.
  • Work with residents and other staff members to maintain safety, cleanliness, and overall appearance of interior/exterior of facility.
  • Report volunteer needs to Volunteer Coordinator.
  • Assist with supervision of volunteer projects as needed.
  • Other duties as assigned

Education and/or Experience

  • Undergraduate degree in Social Work preferred.
  • A minimum of 2 years experience in adult case management preferred.
  • Must have valid driver's license and auto insurance.
  • Clearance of background check and drug screen.

Skills and Abilities Required

  • Clear, oral and written communication skills.
  • Basic knowledge of computer literacy skills.
  • Patience when assisting population served.
  • Ability to handle multiple projects and tasks simultaneously.
  • Ability to work as cooperative and supportive team member
  • Knowledge of safety policies and procedures.  Adherence to policies and procedures are a must.
